Halloween for Charity Mini Sessions 2018 Sign Up Form

Please fill out the form if you are interested in scheduling a Halloween for Charity Mini Session on Sunday, October 21st. Once I receive your form, I will email you a link to sign the contract for your session. A signed contract with the model release section checked is required to participate in this event. No purchase is required but all participants must bring in a 3 item donation for the Palatine Food Pantry for each space they take. All participants of this event will receive a FREE 5 minute session with their donation. Each family can reserve one space for each of their children. More than one child can participate and each will receive 2-3 proofs in their online gallery. PLEASE FILL OUT ONE FORM FOR EACH CHILD PARTICIPATING! No sibling images will be taken for this special event due to time restraints. All prints, digital files and custom products will be sold separately but at discounted pricing. Price start at $25 for a single digital file and there are other printed products available.

Children participating must be between the ages of 2 months and 12 years old. All children participating must be in costume for the event.
